Thank you for your interest in the PC-API initative!

Purpose of the Initiative:

Performance indicators are used in health care to measure various aspects of quality of care such as prevention, health promotion, disease management, and health care provider interaction with patients. This inititative provides primary care sites with an evidenced-based asthma indicators tool for recording information to assist them in monitoring the management of their patients with asthma and/or chronic obstructive pulmonary disease (COPD).

Background:

15 primary care asthma performance indicators (PC-API) were derived using a modified Delphi approach with a multi-discipline expert panel. The PC-APIs are presented in 9 main questions. Feasibility of using the PC-API form was tested and demonstrated (with 5 sites, 50 prospective patients and 50 retrospective charts).

Description of the Project:

The initiative includes primary care sites from across the global community. Each participating health care practitioner fills out the PC-API form during or immediately following a clinic visit of a patient with asthma and/or COPD. The information captured in the PC-API form (i.e. the indicators) are entered into the Research Electronic Data Capture system (REDCap), a secure server hosted by SickKids. Each form takes approximately 10 minutes to complete.

Benefit:

In participating in this project you will receive individualized quarterly report summarizing performance on each indicator in comparison to all of your country-specific sites combined. No individual patient level data will be reported. Sites will be entitled to use their own site and/or country-specific data for reporting or publication. Aggregated data from other countries will require permission from respective countries.

Please find attached a sample PowerPoint Presentation demoing the initiative, a sample report for your review, and consent form should you wish to participate. If you require further information or have any questions please feel free to contact teresa.to@sickkids.ca.
