Study summary: Ensuring high quality and successful transition for youth with chronic conditions remains a challenge with limited interdisciplinary and intersectoral between the pediatric and adult care settings. The primary goal of this project is to enhance our understanding of the lived experiences of Canadian youth with chronic physical and mental conditions as they transition into adulthood and adult healthcare services. Using a narrative photovoice approach, we will illuminate these transition experiences and co-design journey maps with youth and their caregivers to identify system-based solutions to facilitate successful transitions.
