For some children, there may be 6 additional visits in the first year. These tasks provide information about your child\u2019s thinking abilities. Some tasks are on the computer while other tasks involve looking at pictures, completing puzzles, answering questions, or using a paper and pencil. Your child will also be asked to fill out questionnaires asking about their own feelings and behaviour.<\/li>\n<\/ol>\n<p>[\/vc_column_text][\/vc_column_inner][\/vc_row_inner]\n\t\t\t<\/div>\n\t\t\t<div  class=\"ult_tabitemname\"  >\n\t\t\t\t[vc_column_text]<\/p>\n<ol start=\"2\">\n<li>You and your child will be asked to complete 20 <strong>questionnaires<\/strong>. The questionnaires evaluate your child\u2019s thinking and problem-solving abilities, behaviour, and functioning across a range of areas (e.g., paying attention, performing in school, completing tasks of daily living). Some of the questionnaires you will be asked to complete ask about your child\u2019s development, health, and family history (such as current medications, academic services, family size).<\/li>\n<\/ol>\n<p class=\"tab\">Also, we will ask for your permission to give a <strong>questionnaire to your child\u2019s teacher<\/strong>. This will help us get ratings of your child\u2019s behaviour in the classroom. In order for us to contact your child\u2019s teacher, you will need to provide us with a signed permission form. Please note that even if you sign this consent form to participate in the study, you have not given us permission to contact the teacher. That process is separate from this consent process.<\/p>\n<p>[\/vc_column_text][vc_column_text]<\/p>\n<ol start=\"3\">\n<li><strong>Symptom assessments<\/strong>, that take place at up to ten (10) time points, assess your child\u2019s experience of physical symptoms (e.g., pain, nausea) and feelings (e.g., worry, sadness) that can happen during and after treatment. You and your child (if your child is old enough) will complete these items on paper or on a computer or tablet.
